Enhanced Care Management (ECM)

At Stay Focused, our Enhanced Care Management (ECM) program is designed to walk alongside individuals facing complex life challenges, such as involvement in the justice system, housing instability, substance use, or serious mental health conditions.

We believe in meeting people where they are. That’s why ECM offers hands-on, personalized support at no cost. Participants are partnered with a Lead Care Manager who takes the time to understand their goals and helps build a care plan that reflects their specific needs. From navigating services to staying on track with appointments, we’re here every step of the way.

What makes us different? We’re rooted in the Kern County community, with deep connections and years of experience helping people overcome obstacles and build lasting change. Our focus is clear: reduce cycles of incarceration and homelessness, prevent unnecessary ER visits, support recovery, and open doors to employment and opportunity.
